Go to Filter > Noise > Add Noise and use the settings in the picture.

Use Filter > Blur > Motion Blur. Make a rectangle and go to Select > Modify > Smooth and choose 20 pixels. Copy it and delete the rest of the layer.

Apply Bevel and Emboss

Go to layer style and use these settings.

Create a hole in your dogtag.

Make it darker with curves (CTRL+ALT+M).

Type your text in grey and use these settings.
